I am requesting that WSAV provide me with a copy of the following story originally aired on NEWS 3, as detailed below. I understand that my $10 non-refundable dub fee is being considered a contribution to WSAV's "Holiday Toy Station" campaign. I further understand that the material I am requesting is copyright protected by WSAV Television, Inc. I will only use this material for my own personal viewing in my own home. I will not use this material in any courtroom, nor will I broadcast this material for others to see in any way.

To request a tape, email Randy Tatum at rtatum@wsav.com. Please include:

· your contact information,
· the date and time (morning, 6 or 11) the show aired, and
· a brief description of the segment you want.

Also specify if you want the tape mailed to you or ready for you to pick up at the station.
